im supprised none caught on that acromegaly is enlargement of hands, feet and face.

That's what I got from the wikipedia link. The transsphenoidal surgery involves going through the upper gum into the pituitary, or alternatively through the nose or under the nose. Not technically brain surgery but still entails some mucking about in your head.
